How it Works

Select the organisations/departments you wish to change your name with.
Provide certified copies of documents (e.g. Marriage Certificate, Divorce Certificate or Name Change Certificate, current Passport and current Drivers License) where required.

Post completed forms and Certified copies of your certificates to individual organisations (Standard & Premium packages) or back to us (Deluxe package).
